Suicides that became unintentionol mass murder

Some suicides have unintentionally ended up with mass murder

This incident happend not far from Hungerford, and not far from where I was born.

Philippe Autrut unintentionally killed two people and wounded 13 others after he lit a cigarette that then caused an explosion when he was attempting to asphyxiate himself with gas jets in Malakoff, Paris in 1986. And he survived his suicide attempt!

Looks abit like Aaron Alexis, i could be wrong though

Not completely relevent, but I found this a month or so ago about people who attempted suicide but accidently killed others in the process.

Its a facial reconstruction of a unidentified body found in New York in 1997. He was identified in 2018.

There's a man named Khun Jalaepat in this list that attempted to drown himself but his friends attempted to save him and drowned, and 4 were killed, while he survived. Basically we have a new example.

It may not count, but your question made me think about the Germanwings Flight 9525. I am not sure it counts because the co-pilot crashed the plane to commit suicide, and thus also killed all 144 passengers and 6 members of the crew, but he knew he would kill them, so it is not really unintentional. I don't know enough the case to know if he wanted the others to die, or if only wanted to commit suicide.
